Each entry is represented by two numbers separated by a colon, followed by two other numbers separated by a colon. The first set of numbers is the scriptural reference in Section 135 of the Doctrine & Covenants, and the second set of numbers is the volume and page number in Answers To Gospel Questions where the indicated scripture is mentioned and discussed. For example, [ D&C 135:3 . . . 5:184 ] would indicate that D&C 135:3 is discussed in volume 5, page 184.
